The Registered Dietitian works collaboratively with medical providers, nursing, behavioral health, pharmacy, and other members of the healthcare team to develop individualized nutrition care plans, improve patient outcomes, and address barriers that may affect a patient’s ability to achieve nutrition and health goals. This position also supports preventive health, chronic disease management, quality improvement, and community health initiatives.

·         Maintain regular outreach and follow-up with patients, including those identified as high risk for substance use, suicidality, or self-harm.

·         Provide psychoeducation to patients and families regarding behavioral health diagnoses, treatment options, medications, and recovery resources.

·         Connect patients with community-based resources and assist with accessing social services, financial assistance, housing, transportation, and other support services as appropriate.

·         Coordinate referrals, prior authorizations, and follow-up appointments with internal providers and external community partners.

·         Assist patients in identifying goals, developing action plans, and overcoming barriers to treatment and recovery.

·         Participate in ongoing communication with case management staff, utilization management staff, health plan partners, and community providers to support coordinated care.

·         Maintain accurate, timely documentation within the Electronic Health Record (EHR).

·         Assist with the implementation of organizational policies, procedures, and quality improvement initiatives related to case management and behavioral health services.

·         Ensure compliance with HIPAA, organizational policies, and applicable state and federal regulations.

·         Participate in required trainings, staff meetings, and continuing education activities.

·         Perform other duties as assigned.

Education & Experience

·         Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related behavioral health field required. Master's degree preferred.

·         Minimum of one (1) year of experience in case management, behavioral health, counseling, healthcare, social services, or a related human services field required.

·         Experience working with individuals experiencing mental health and/or substance use disorders preferred.

·         Experience working in a Federally Qualified Health Center (FQHC), community mental health setting, integrated primary care setting, or similar healthcare environment preferred.

·         Certified Case Manager (CCM) credential is a plus but not required.

·         Strong empathy, active listening, and patient advocacy skills.

·         Knowledge of behavioral health conditions, co-occurring disorders, and substance use disorders.

·         Familiarity with community resources, support services, and referral networks available to assist patients with healthcare and social needs.

·         Knowledge of Social Determinants of Health (SDOH) and their impact on patient outcomes.

·         Strong organizational and time management skills with the ability to prioritize multiple responsibilities in a fast-paced healthcare environment.

·         Ability to collaborate effectively with multidisciplinary healthcare teams and community partners.

·         Proficiency in Microsoft Office and Electronic Health Record (EHR) systems.

·         Knowledge of HIPAA regulations and the ability to maintain the confidentiality, security, and integrity of patient information.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Drives the mobile vehicle(s) in a safe and timely manner to various locations across communities that HCPSC provides programs and services.
• Routes and destinations may change daily based on program and service locations.
• Conducts daily safety inspection of mobile vehicle(s); inspect the interior, exterior, brakes, headlamps, taillights and report mechanical malfunctions or other problems as required.
• Responsible for daily vehicle cleaning, ensure routine fueling, check oil and fluid levels, clean windshield, and headlamps, and maintain mobile vehicles in clean and safe operating condition.
• Keep mobile vehicles maintenance records current; coordinate and schedule routine and episodic vehicle maintenance.
• Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards, i.e., vehicle logs.
• Performs other duties as assigned.

Education/Certification/Licensure:

• High School Diploma or GED equivalent required.
• Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
• Requires an active South Carolina driver's license for the class required for operation of assigned vehicle; DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
• CDL is required

Experience/Specialized skills (including Language)

• 1-year related experience preferred.
• Possess safe and good driving skills.
• Knowledge of the South Carolina area and surrounding communities.
• Bilingual in English/Spanish preferred.
• Intermediate interpersonal and customer service skills.
• Must be able to communicate with and relate to a diverse staff and clients in a courteous and professional manner.

Ability to maintain vehicle records and perform routine checks on a motor vehicle
